Pressure sore! A scary word for every wheelchair user and a condition we all want to avoid.
And even though I thought I was doing my best not to have them, one day it started.....a dreaded pressure sore. Pressure sores may sound innocent, but they can lead to infection. And if it gets bad enough, sepsis can set it. Many wheelchair users are hospitalized every year and many die from it. So, I take pressure sores very seriously.
My pressure sore started with an innocent scab, which, when it fell off, left behind a wound. Although we took immediate steps when we noticed, it did not go as we wanted. The wound did not heal but only grew and deepened. At first, I didn't understand why it happened, but it turned out I was sitting on the wrong cushion.
The wound was about the size and depth of a beer bottle stopper but to me, it already looked like a crater. Since I couldn't take care of it myself, my mother did everything. Thanks to Mom!! And to a friend who has experience dealing with pressure sores who gave me a lot of advice. And yes, I didn't go to my doctor. In the beginning, I was thinking that all of this would soon pass. Then I just asked to be seen by the nurse in the department where I work as an occupational therapist. I had to wait a long time for an appointment to the doctor.
So how long do you think the treatment lasted? Five months! In the beginning, I did not attach much importance to this situation. When the wound started to get bigger and deeper - I was scared. I was
worried all the time. But I was calmed down by the nurse who helped treat the pressure sore and finally I was examined by a doctor who prescribed patches for pressure sores. First, we treated the pressure sore with polarized light therapy and with patches, then I changed my wheelchair cushion. For about a week I stayed in my bed, shifting from side-to-side. Of course, obvious things like wound cleaning we did as well. And this went on for five long months. I was also tortured by autonomic dysreflexia (https://www.christopherreeve.org/living-with-paralysis/health/secondary conditions/autonomic-dysreflexia ), a very dangerous condition caused by the pressure on the sore.
I'm glad it finally healed. And even though I thought I knew all the principles of how to prevent bedsores, well I got one anyway. .. so if you don't know or have forgotten what to do, let's repeat it.
1. Most important is to use a good quality wheelchair cushion. There is no right cushion for everyone.But switching to an air cushion helped me a lot.
2. A well-adjusted wheelchair is also very important for proper seating, which avoids unnecessary
pressure
3. Shifting positions while sitting in a wheelchair is the best method for avoiding pressure sores. 
4. Check daily for areas that are prone to pressure sores (sacrum, heels, etc.)
5. Look for discolored skin. If you find a spot that is red, swollen or just different than the rest of the around, be cautious and keep off of it for a while until it recovers.

                               Mano patirtis gydantis pragulas

Pragula! Baisus žodis kiekvienam neįgaliajam ir būklė, kurios visi neįgalieji norime išvengti.
Ir nors aš galvojau, kad darau viską, kad jų neturėčiau, vieną dieną tai prasidėjo..... Pragula. Iš pradžių tai gali pasirodyti kaip nekalta žaizdelė, tačiau jos gali sukelti infekciją ar net gali isivystyti sepsis. Daugelis neįgaliųjų kasmet paguldomi į ligoninę gydytis pragulų ir daugelis nuo to miršta. Taigi aš labai rimtai vertinu atsiradusią pragulą.
Mano pragula prasidėjo nuo nekalto šašo, kuris, nukritęs, paliko žaizdą. Nors pastebėję ėmėmės neatidėliotinų žingsnių, tačiau viskas vyko ne taip, kaip norėjome. Žaizda negijo, o tik didėjo ir gilėjo. Iš pradžių nesupratau, kodėl taip atsitiko, bet paaiškėjo, kad sėdžiu ant netinkamos pagalvėlės.
Žaizda buvo maždaug alaus butelio kamštelio dydžio ir gylio, bet man ji jau atrodė kaip krateris. Kadangi pati atsiradusia pragula negalėjau pasirūpinti, mama darė viską, kad tik greičiau gytų. Ačiū mamai!! Draugė, turinti patirties gydant pragulas taip pat davė daug patarimų. Ir taip, aš iš karto nesikreipiau pas gydytoją. Iš pradžių galvojau, kad visa tai greitai praeis. Tada aš tiesiog paprašiau, kad mane apžiūrėtų slaugytoja skyriaus, kuriame dirbu ergoterapeute. Taip pat teko ilgai laukti vizito pas gydytoją.
Taigi, kiek laiko, jūsų manymu, gydymas truko? Penkis mėnesius! Iš pradžių aš šiai situacijai neskyriau didelės reikšmės. Kai žaizda pradėjo didėti ir gilėti - išsigandau. Aš visą laiką nerimavau. Bet mane nuramino slaugytoja, kuri padėjo gydyti pragulą, ir galiausiai mane apžiūrėjo gydytoja, kuris paskyrė pleistrus pragulai gydyti. Pirmiausia pragulą gydėme poliarizuotos šviesos terapija, tada klijavome prieš pragulinius pleistrus, tada aš pakeičiau pagalvėlę ant kurios sėdžiu. Maždaug savaitę pragulėjau lovoje besisukiodama nuo šono ant šono. Žinoma, mes darėmeir tokius akivaizdžius dalykus kaip žaizdos valymas. Ir tai tęsėsi penkis ilgus mėnesius. Mane taip pat kankino autonominė disrefleksija (https://www.christopherreeve.org/living-with-paralysis/health/secondary rights / autonomic-disreflexia), labai pavojinga būklė, kurią sukelia pragulos spaudimas. Džiaugiuosi, kad pagaliau pragula sugijo. Ir nors maniau, kad žinau visus principus, kaip užkirsti kelią praguloms, vis tiek aš ją pasidariau.. Taigi, jei nežinote ar pamiršote, ką daryti, pasikartokite:
1. Svarbiausia yra naudoti geros kokybės ir jums tinkamą pagalvėlę. Vienos geriausios ir tinkamiausios pagalvėlės visiems - nėra. Tačiau man labai padėjo kai atsisėdau ant orinės pagavėlės.

2. Tinkamai sureguliuotas neįgaliojo vežimėlis padeda ivengti bereikalingo spaudimo.
3. Padėties keitimas sėdint vežimėlyje yra geriausias būdas išvengti pragulų. 
                 
4. Kasdien tikrinkite, ar nėra įbėžimų, žaizdelių ar pan.  (ant kryžkaulio, kulnų ir kt.)
5. Stebėkite odos spalvą. Jei radote dėmę, kuri yra raudona, patinusi ar tiesiog kitokia, kreipkitės į gydytoją dėl gydymo arba stebėkite patys.
